This project will contribute to reducing maternal and child deaths within a total population of 18,000 living in poverty in the Karnali Region, West Nepal. Poor health is linked to malnutrition and lack of access to health services; we will improve access to nutritious foods and basic health services, specifically for mothers and children (but also everyone else in the community). Support to government health services, health education and work with women’s groups will ensure people have access to and use health services appropriately and attend early with treatable illness; preventative services will help reduce illness. Food security will be improved through seed and stock distribution, access to better stud goats and training for male and female farmers in animal fodder production, soil management, irrigation methods and vegetable / vegetable seed production. ;

The proposed second phase of this project will further cover around 20,000 beneficiaries in the same districts, through construction of 15 community water supply schemes and WASH facilities in 10 remote schools covering most vulnerable areas of 4 Rural Municipality (6 VDCs). This phase of project considers the evolving context of these two districts (two years after the earthquake) by taking into account the aspirations of people living in the area. The constraints posed by time and funds during the 1st phase are well-thought-out to ensure that this phase of project would leave no one behind. This project employs participatory methodologies that promote inclusion and community decision making, and is building the capacity of community based platforms including users committee, health management committee and school management committee. Considering the new federal structure in Nepal - Action Against Hunger and its local partners are building dialogue between communities and local government in order to increase government support for WASH sector and investment for sustainability of the WASH structure in the long run. Apart from this, this project will contemplate the integration of WASH and nutrition specially targeting the Severe/Acute malnourished children along with under 2 year old children from the targeted areas by implementing intensive WASH behaviour change related activities. Expected results: Unreached earthquake affected as well as nutritional vulnerable population have improved access to safe and sufficient water. The proposed project aims to strengthen Nepal’s tuberculosis (TB) response through an integrated, community-driven approach focused in Madhesh Province, one of the country’s highest TB-burdened regions. Building on CARE Nepal’s long-standing experience in health systems strengthening and partnerships with the Global Fund and local governments, the project will improve TB prevention, detection, treatment, and governance. Over three years, the partnership will enhance service delivery in health facilities, mobilise frontline health workers and community volunteers, and promote community engagement to reduce stigma and increase treatment adherence. The programme will also leverage CARE’s proven tools such as the Community Health Score Board (CHSB), Social Analysis and Action (SAA) approach, and the HEAL Hub digital platform to improve accountability, empower communities, and scale digital and AI-enabled innovations for TB detection and adherence monitoring. By embedding resilient health systems and linking TB care with early warning mechanisms, nutrition, and livelihoods, the project will contribute to sustainable, equitable health outcomes and strengthen local governance through the institutionalisation of the TB-Free Municipality initiative in line with Nepal’s national TB strategy.

Province 7 is one of the most remote and poorest areas of Nepal (HDI: 0.31) with 62.5% of population living in poverty (CBN) and ranked most vulnerable in Nepal (DFID 2017). The project will aim to reduce maternal mortality rates and improved local structures of maternal healthcare provision for 1,000 women in two communities in Province 7 - Rugin and Bichhaya - and improve agricultural productivity and production and income for vulnerable women, by empowering 625 women through productive resources, inputs, confidence, and knowledge. The project will support poor and vulnerable families (including infants) to have better access to safe, nutritious food all year round and improved resilient agricultural practices which will strengthen capacity for adaptation to climate change, extreme weather, and other disasters.
