Countries have complete control over how and when to use their ISS funding on the condition that DTP3 coverage rates continue to rise. One of the first performance-based programmes of its kind, the ISS programme has two main characteristics aimed at supporting countries to increase their immunisation coverage.

Gavi supports JE catch-up campaigns and co-finances the vaccine for routine immunisation. In 2014, Gavi invited countries to apply for support to introduce the Japanese encephalitis (JE) vaccine. The first Gavi-funded JE vaccine campaign started in Lao People’s Democratic Republic in 2015, with Nepal and Cambodia following suit in 2016. Myanmar conducted a nationwide campaign targeting children 9 months–15 years in 2017 with introduction into routine nationwide in 2018, and Bali in Indonesia conducted the islandwide campaign in 2018 with introduction into routine in the same year.

The Vaccine Alliance supported one of the fastest roll-outs in the history of vaccination: the introduction of at least one dose of IPV into the routine immunisation schedules of Gavi-supported countries, which was achieved in April 2019, less than five years after the initial launch. Countries introducing IPV received a one-time cash grant from Gavi, funded by GPEI, to support some of the additional activity costs related to introducing the new vaccine. These activities may include health worker training, social mobilisation, micro-planning, expansion or rehabilitation of cold chain equipment, printing of materials (such as immunisation cards), technical assistance and modifications to the surveillance systems.

GAVI’s support for new vaccines presents an opportunity for countries to not only implement vaccines that protect against diseases causing substantial morbidity and/or mortality, but also to build upon the vaccine investments in order to increase immunisation coverage rates, and strengthen routine immunisation services and primary health care systems. In line with a 2009 WHO position paper on cervical cancer recommending that routine human papillomavirus (HPV) vaccination be included in national immunisation programmes, GAVI provides support to eligible countries for HPV vaccines that protect against types 16 and 18, responsible for causing about 70% of cervical cancer cases. The form of support provided by GAVI depends on the level of demonstrated ability in a country to deliver vaccines to young adolescent girls. Countries that have demonstrated the ability may apply for GAVI support for nationwide introduction of HPV vaccines.

As such, it is necessary for applications for HSS support by countries to be based on a strong bottleneck and gap analysis, and present a clear results chain demonstrating the link between proposed activities and improved immunisation outcomes. GAVI’s approach intends to deliver and document results. From 2013, the HSS cash-based funding stream replaces the previous support to Immunisation Services Support (ISS), Health Systems Funding Platform (HSFP) and Civil Society funding.

Combining five different vaccines in a single vial, pentavalent vaccine brings cost savings in terms of equipment, delivery and disposal programmes. When Gavi set out to catalyse the adoption of new vaccines in low-income countries in 2000, Hib was top of its priority list. The vaccine had existed since the early 1990s, but by 2000, only one low-income country had introduced the vaccine into its national immunisation programme. As a result, support for routine immunisation against Hib was immediately made available through Gavi. Initially, Gavi-eligible countries were slow to take up the offer of support for Hib vaccine, both because of limited awareness of Hib's disease burden and concerns about the cost of the vaccine. By 2004, four years after the Hib vaccine had become part of Gavi's vaccine portfolio, only 15 countries out of the 74 eligible countries had been approved for Hib vaccine support. Initially, Gavi provided support for hepatitis B (hepB) monovalent vaccine. However, in December 2005, the Gavi Board decided only to accept new applications for hepB vaccine support through combination vaccines (tetravalent and pentavalent).

Since April 2020, NYF has launched effective emergency programs in Nepal to fight COVID-19. We served 5,311 free meals and distributed over 5,061 kg of vitamin-packed "super flour." Our Access to Education program opened 80 school centers and provided education to over 8,000 students. Our emergency women's home sheltered 38 people, and our COVID Isolation Center is still providing services to infected patients. The crisis isn't over yet, and our responses are adapting as the situation unfolds.
