The proposed project aims to strengthen Nepal’s tuberculosis (TB) response through an integrated, community-driven approach focused in Madhesh Province, one of the country’s highest TB-burdened regions. Building on CARE Nepal’s long-standing experience in health systems strengthening and partnerships with the Global Fund and local governments, the project will improve TB prevention, detection, treatment, and governance. Over three years, the partnership will enhance service delivery in health facilities, mobilise frontline health workers and community volunteers, and promote community engagement to reduce stigma and increase treatment adherence. The programme will also leverage CARE’s proven tools such as the Community Health Score Board (CHSB), Social Analysis and Action (SAA) approach, and the HEAL Hub digital platform to improve accountability, empower communities, and scale digital and AI-enabled innovations for TB detection and adherence monitoring. By embedding resilient health systems and linking TB care with early warning mechanisms, nutrition, and livelihoods, the project will contribute to sustainable, equitable health outcomes and strengthen local governance through the institutionalisation of the TB-Free Municipality initiative in line with Nepal’s national TB strategy.
