F.a: Maithili Empowerment Programme

Maithili Empowerment Programme. The programme seeks to increase a local partner’s capacity to respond to the needs of the Maithili minority in Southern Nepal. The 2.5 million Maithili of Southern Nepal make up the country’s largest ethnic minority. For merly known for their rich culture and traditions, the Maithili now live in poverty and have almost lost their unique identity. To address the needs of the Maithili, Fida partners with a local NGO called Relative Nepal, which belongs to the Assemblies of G od Nepal (AGN) church. The programme offers the partner hands-on training in community development work. The main approach used is that of Community Health Education. The work carried out among the Maithili communities includes e.g. literacy training, basi c education and cultural revitalization activities. Through the trainings and practical development work conducted during the programme, the partner’s capacities build up and, at the same time, the needs of the Maithili communities are addressed. Already one school has been established and children’s literacy classes started. These together will contribute towards the improvement of the quality of life in the Maithili communities.

Empowering Partners for Social Responsibility. The project aims to increase the capacity of a local partner to do holistic development work in local communities. Nepal is one of the poorest countries in the world, where majority of the people live in rural areas. Fida’s partner in Nepal is the Assemblies of God church (AOG). The project provides training and awareness-raising for the partner on a great variety of issues related to development work. Fida and AOG work together in the local communities and t hrough this hands-on development work the partner acquires practical skills. The project focuses on three major components: Literacy, Community Health Education (CHE) and HIV/AIDS. As AOG’s skills and knowledge grow, the partner will become increasingly capable of doing effective community development work of its own. This will contribute to the increasing impact of the partner’s work at community level.

Research and policy coalition for reduction of sugar-sweetened beverage consumption in Bangladesh, Nepal, Pakistan and Sri Lanka

The nutrition transition in South Asia, both in the rapidly urbanizing population and in peri-urban and rural areas, has led to diets that include a high amount of ultra-processed food, including sugar-sweetened beverages (SSBs). Aggressive marketing of SSB industries, along with inadequate policy, low compliance and weak enforcement of existing policies, is leading to high consumption of SSBs, particularly among children and youth in the countries of focus. Evidence from global research suggests that SSB labels, both text or image-based, reduce young people’s predicted preferences for, and reported probability of purchasing, SSBs. This also influences parents’ willingness to purchase SSBs for their children. Calorie information may also reduce selection and consumption, specifically of SSBs. This project seeks to build momentum for a more effective regulatory and fiscal policy in Bangladesh, Nepal, Pakistan and Sri Lanka and, ultimately, across the region. The project will undertake analysis of current policies and laws as well as SSB marketing practices and then co-create culturally relevant and understandable package labels. This will serve as the basis for policy recommendations for contextually appropriate labelling to inform consumer choice and improve diet-related health outcomes. This project is part of a cohort supporting diverse and multi-actor coalitions that combine collaborative learning networks and communities of practice, operating at multiple scales, to collectively support the production, consumption of and access to healthy diets for all, particularly the most vulnerable, emphasizing sustainable systems that contribute to the health of people and ecosystems.

Building a South Asian coalition for food systems transformation through fiscal and trade policy

Trade measures, taxes and subsidies are an important means of enabling access to healthy dietary choices. Ensuring policy coherence, such as aligning domestic agricultural subsidies, taxes and trade policies with interventions for better nutrition, is a complex undertaking. This project will address the research gap on how fiscal and trade policies interact with dietary patterns and consumption choices to impact nutrition and health outcomes. This project aims to develop a South Asian research, policy and practice network for deploying fiscal and trade policies to improve access and affordability of healthy diets. The project team will use scoping reviews and exploratory research techniques to develop country-specific analysis to identify and use fiscal and trade measures to overcome the triple burden of wasting, stunting and obesity, and to promote healthy diets. This analysis will include an assessment on how national food safety requirements are connected to trade policies. The research team will focus on understanding how policy reform can address gender disparities, which are one of the most pervasive forms of inequality and a major cause of poor health and nutrition outcomes in South Asia. This project is part of a cohort supporting diverse and multi-actor coalitions that take on a leadership role to influence policies and transform food systems at the country and regional level in Central and South America, South Asia and Southeast Asia. These coalitions combine collaborative learning networks and communities of practice, operating at multiple scales, to collectively support the production and consumption of, and access to, healthy diets for all, particularly the most vulnerable, emphasizing sustainable systems that contribute to the health of people and ecosystems.

Scaling responsive and responsible artificial intelligence for improving sexual, reproductive and maternal health in South Asia

IDRC is embarking on a five-year partnership (Artificial Intelligence for Development: Responsible AI, Empowering People) with the United Kingdom’s Foreign, Commonwealth and Development Office, also known as AI4D-FCDO, to support policy, innovations and expanded leadership to spur safe and responsible artificial intelligence (AI) development in Africa and beyond. The vision of the program is to support an inclusive and responsible AI ecosystem that improves the quality of life for all in low- and middle-income countries. This project, which is part of AI4D-FCDO, will ensure that sexual, reproductive and maternal health (SRMH) hubs around the world graduate from proof-of-concept to testing and scaling. In South Asia, the AI Sarosh hub has awarded nine grants in Bangladesh, Nepal, Pakistan and Sri Lanka. The grants cover areas such as adolescent health, developing affordable colposcopy imaging and digital microscopes, early detection of postnatal depression, risk management for pregnant mothers, AI-enhanced fetal ultrasound services, AI-enabled detection of perinatal depression and early diagnosis of gestational diabetes. The monitoring of these projects has allowed AI Sarosh to identify gaps in policy and regulatory frameworks, implementation challenges, and a lack of an evidence base on the responsible use of AI in health in the region. Through this project, AI Sarosh aims to: generate evidence by supporting field evaluations of the solutions currently in development in real-life settings that can inform scaling; improve understanding of SMRH issues and the potential of AI applications in SRMH through gap analysis; develop strategies to inform policy and regulatory decision-makers and relevant stakeholders; and provide a platform for the sub-grantees to interface with other health and AI experts.

AI4D-FCDO Innovation: Scaling and sustaining AI solutions to improve sexual, reproductive and maternal health in sub-Saharan Africa

In sub-Saharan Africa, indicators for maternal, sexual and reproductive health continue to lag other health indicators. For example, the continent is not on track to achieve the Sustainable Development Goal’s target of ensuring universal access to sexual and reproductive health-care services. Artificial intelligence (AI) holds the potential to be a transformative tool in tackling the continent’s ongoing maternal, sexual and reproductive health challenges. However, significant concerns remain about the potential harms and risks of using AI in this sector. IDRC supported the establishment of a Hub for Artificial Intelligence in Maternal, Sexual and Reproductive Health in sub-Saharan Africa, which, among other things, has funded development of a tool for predicting miscarriages among women in Uganda. In this new project, the hub seeks to scale a subset of promising AI innovations for maternal, sexual and reproductive health from the previous project. The new project will also strengthen the capacity of AI innovators to create solutions for local maternal, sexual and reproductive health priorities based on large datasets. By the end of the project, the project teams aim to have AI solutions scaled in five African countries and four new AI innovations developed through an open innovation competition. IDRC is supporting a five-year partnership, called Artificial Intelligence for Development: Responsible AI, Empowering People, with the United Kingdom’s Foreign, Commonwealth and Development Office to support policy, innovations and expanded leadership to spur safe and responsible AI development in Africa and beyond. The vision of the partnership is to support an inclusive, responsible AI ecosystem that improves quality of life for all.

In the southern Terai, extreme heat and humidity are driving rapid growth in energy demand for cooling, increasing emissions and placing significant stress on energy infrastructure. Dhangadhi Sub-Metropolitan City (DSMC) in Sudurpaschim Province experiences recurrent heatwaves during May–August, with temperatures frequently exceeding 40°C. Climate variability, rapid and unplanned urbanization, loss of green spaces, and limited energy-efficient infrastructure exacerbate risks to public health, livelihoods, and essential services, disproportionately affecting low-income households, outdoor workers, and residents of poorly insulated housing. Impacts include heat-related illness and mortality, school disruptions, and reduced functionality of public facilities. Strengthening Health Resilience in Nepal - HIV, TB and Malaria-UNDP Contribution

The Strengthening Health Resilience in Nepal - UNDP- HIV/TB and ML Project (the Project) will support the Government of Nepal (GoN) to strengthen the country’s resilience and health response and contribute towards a world free of the burden of HIV/AIDS, TB, and malaria with better and equitable health care for all. The project’s focus is to address the significant gaps in TB case notification by maximising efforts to reach missing cases with focus on screening among key and vulnerable populations and engaging private service providers. There is a need to improve the capacity of lab network especially in optimising sample transportation, diagnosis, utilisation of GeneXpert and ensuring quality assurance across the diagnostic lab network. Further, the Project will focus on expanding the treatment coverage for DRTB cases and bringing initial loss to follow up cases to radar of health care and treatment. Nepal’s shift to a federal system of governance, as outlined in the Constitution of 2015, has affected human resource capacities of TB and HIV/AIDS programmes, in particular at provincial and local levels. The Project aims to address this by supporting capacity development relevant stakeholders at the sub-national levels, thereby improving resilience of health systems to effectively response to HIV/TB and ML in the country.

Strengthening Health Resilience in Nepal - HIV, TB and Malaria

Nepal faces significant challenges in combating HIV, Tuberculosis (TB) and Malaria, despite noteworthy progress and achievement in public health and country’s response to health issues. According to the National Center for AIDS and STD control (NCASC), approximately 30,300 people live with HIV with concentrated prevalence among key populations such as sex workers, people who inject drugs, men who have sex with men and migrants who face higher risks due to unsafe practices and lack of targeted interventions. In 2023, 457 people were infected with HIV, a 91% reduction of annual infection rate compared to statistics in 2000. Whereas TB remains a major concern with an estimated incidence of 69,000, with only 69% of infected persons notified,1 and mortality among HIV negative persons of 17,000 Drug resistant TB (DRTB) is an emerging concern, further complicating prevention and treatment efforts. Nepal has started to implement a TB free initiative which has the potential to change the trajectory of the TB burden in the country. However, the initiative is currently limited to 150 out of 753 local governments. Limited or overburdened human resource capacity has markedly reduced the capacity to coordinate the TB programme implementation at the provincial level, and capacity development needs remain high. Further, the weak maintenance system of equipment including calibration, updating software versions and timely repair of equipment and diagnostics further challenge the disease treatment in Nepal. For the last decade Malaria cases have declined significantly but the disease remains endemic in 25 districts with over 1.2 million people who are at risk due to limited access to healthcare, particularly in remote areas of the country. Socio-economic factors, stigma, and poor health infrastructure all play major roles in timely treatment and access to the services. Enhancing Government's capacity for effective response to HIV and TB in Nepal

Through this Initiation Plan, UNDP Nepal aims to support Ministry of Health and Population, Government of Nepal by strengthening its capacity to combat TB and HIV cases and equipping the relevant entities with health and non-health drugs, diagnostics, and equipment. This Initiation Plan is specifically focused to support the transition of procurement of drugs, diagnostics, and equipment under the costed extension of Global Fund Grant Cycle 6, to be implemented by Save the Children and funded by the Global Fund.
